Transition from Pediatric to Adult Care in Adolescents with Chronic Illness: Psychosocial Factors, Self-Efficacy, and Innovative Interventions
The transition from adolescence to adulthood is a particularly relevant developmental stage for the consolidation of health habits, the development of personal autonomy, and the gradual assumption of self-care responsibilities. In adolescents and young people with chronic illness, this stage gains added complexity as it coincides with the planned move from pediatric to adult healthcare services.
Healthcare transition does not simply mean a change in professionals or care setting, but a profound reorganization of roles among adolescents, parents, and healthcare teams. This thematic panel proposes addressing this transition from a psychosocial, developmental, and innovative perspective, integrating four complementary contributions: a systematic review of psychosocial factors and transition interventions; analysis of the mediating role of self-efficacy in adolescents and parents; study of the subjective experience of transition and its associated determinants; and evaluation of the ERES App as a digital intervention, along with an improvement proposal based on adolescent-centered design and the incorporation of a virtual assistant.
